Print all pages on single-sided A4 paper. Do not staple.
Sign in Order

Post to OPG
Send signed forms to the Office of the Public Guardian.
Wait for Registration
OPG registration takes 8-10 weeks. They'll return your registered LPA.

Fee Exemptions & Reductions
